If you have a signed L4 mediated agreement (Form 200 consent order), you don't need a new application — you file Form L4 with the LTB, attaching proof of breach. The L4 is reviewed administratively (no hearing in most cases) and an order issues quickly, typically within 2-3 weeks.

Specific steps:
1. Document the missed payment: bank record, tenant communication, your written reminder.
2. File Form L4 with the LTB through the portal. Attach the original consent order and proof of breach.
3. Once the order issues, you have 6 months to file with the Sheriff for enforcement.

What to watch: the LTB is reviewing L4s strictly right now — your evidence of breach has to be airtight. A missing 10 days isn't enough on its own; you need a clear pattern that the agreement is broken.
